- How much does it cost?
- The Online Medical Evaluation costs $29 per person, effective February 4, 2017.
- What forms of payment do you accept?
- 3M accepts credit cards (Mastercard, Visa and American Express) and purchase orders. Purchase orders may be used on orders for 50 or more evaluations ($1450). Orders for less than that amount must use a credit card, effective February 4, 2017.
- In what languages do you offer the evaluation?
- Both English and Spanish.
- What if my employees are not computer literate?
- If your employee is able to point and click a mouse, they will be able to complete the evaluation.
- Can I help my employee take the evaluation?
- You can enter the URL into the browser, enter the login ID for them, and show them how to point and click the mouse. Per Appendix C of The OSHA Respirator Standard, the employer or supervisor must not look at or review the employee’s answers.
- How long does it take?
- It takes a typical employee about 20 minutes to complete the evaluation once the account is set up.
- Who reviews the evaluations?
- Board-certified occupational medicine physicians review evaluations requiring further evaluation.
- How long will it take to get the results?
- Most employees who take the online medical evaluation will be cleared upon completion of the test. Employers who opt to be emailed will receive notification within minutes of the test being completed. Those evaluations requiring further evaluation are reviewed within one (1) business day and an employer who opts for email notification will be notified upon completion of the review. Approximately 2% of employees will not be able to be certified by the online system.
- Can I check on the status of my employees' results?
- The Administrative site allows you to view when employees took the evaluation and their status (cleared, pending review, denied, etc.)
- How do I send in follow-up medical information to have an employee’s status updated if he/she is one of the 2% of people that are referred?
- The employee or the provider they see should fax the follow up documentation to us at 612-436-3423. If the employee gives the follow up information to the employer, the employer can upload it (Note: if the documentation contains medical information other than the employee's ability to wear a respirator, it should not be given to the employer for privacy reasons).
- Will I be able to obtain additional copies of clearance letters for myself or my employee if needed?
- The Administrative site allows you to view and print the clearance letters at any time.
- What if one of my employees has a question while taking the evaluation?
- They may call our toll free customer service number at 1-800-383-3393.
- How do I get started?
- In order to get started, Click "NEW ACCOUNT SIGN UP" on our home page. You will first need to make a purchase. There is no minimum purchase amount. Once a purchase is made, you will receive an email with further instructions on how to set up an account. Click here for an Overview of Account Set up.
- My company has used this service in the past, but we cannot remember our login and/or password. What should I do?
- If you know what email address was used, click here. If the email address is no longer available or you do not recall which email address was used, please call our customer service department at 1-800-383-3393.
- Do I have to use a 3M respirator to use this service?
- No, any brand of respirator may be used for the online respirator medical evaluation. You will need to know the type of mask (filtering facepiece, half mask, full face, supplied air, PAPR, or SCBA).
