Medical Questionnaire

How much does it cost?
The On-Line Medical Questionnaire costs $25 per person and the Mail-In Medical Questionnaire costs $35 per person.

What forms of payment do you accept?
3M accepts credit cards (Mastercard, Visa and American Express), purchase orders and EFT (electronic funds transfer).

In what languages do you offer the questionnaire?
Both the on-line and the mail-in questionnaires are available in English and Spanish.

How are the On-line and Mail-In Questionnaires different?
The On-Line Questionnaire is filled out over the Internet, allowing for immediate clearance of approximately 90% of respondents. The Mail-In forms are mailed out the day the order is received. The employees answer the questions and the employer mails the forms back to 3M. We contact the employees to obtain any missing information. The forms are then processed and the results sent via email or fax. Those without either fax or email may have their results mailed to them.

What if my employees are not computer literate?
If you employee is able to point and click the mouse, they should be able to complete the questionnaire over the Internet. The employer may aid them by typing the URL in the browser, entering their login ID for them, and helping them navigate the first pages of demographic information. The employee should have privacy or receive help from someone they are comfortable with when they begin to answer the medical questions.

How long does it take?
The questionnaire averages about 20 minutes per employee.

Who reviews the questionnaires?
Board-certified occupational medicine physicians review questionnaires requiring further evaluation.

How long will it take to get the results?
Most employees who take the on-line medical questionnaire will be cleared upon completion of the test. Those letters are faxed or emailed to the employer within minutes of the test being completed. Those questionnaires requiring further evaluation are reviewed within one (1) business day and a letter is faxed or emailed upon completion of the review. That letter may clear the employee or request further information.

Mail-in medical questionnaires are usually processed within one day of receipt. Questionnaires cannot be processed until profiles are completed (profiles outline the conditions under which the employee wears the respirator), and only completed questionnaires can be processed. Incomplete questionnaires are held until we can contact the employee and obtain the missing information.

Can I check on the status of my results?
The administrator's web page allows you to review all employees, a subset of employees alphabetically, or search for a particular employee. You will see when they took the questionnaire and their status (cleared, pending review, denied, etc). You also may view all employees cleared for a certain profile.

Will I be able to obtain additional copies of clearance letters for myself or my employee if needed?
A: The administrator may go to the administrator's web page at any time and print additional copies of clearance letters.

What if one of my employees has a question while taking the questionnaire?
A: They may call our toll free customer service number at 1-800-383-3393.

I need to use the paper questionnaire, but I need fast results. How do I ensure a speedy process?
Paper forms are mailed out within a day of the request. Please allow 2-3 business days to receive the forms. To ensure prompt processing once you return the forms to us, please make sure you have enclosed the completed profile pages (profiles tell us the work conditions under which the employee wears the respirator) and that your employees have answered all of the questions. We will need to contact the employee to obtain any missing information, which can lead to delays in processing.

How do I get started?

My company has used this service in the past, but we cannot remember our login and/or password. What should I do?
If you know what email address was used, click here, enter the email address that was used to setup the account, and click on the link for the forgotten password. If the email address is no longer available or you do not recall which email address was used, please call our customer service department at 1-800-383-3393.

Do I have to use a 3M respirator to use this service?
No, any brand of respirator can be used for the medical clearance. We need to know only the type of mask ( face filtering facepiece (dust mask), half facepiece, full facepiece, PAPR, SCBA, or supplied air).
